OBJECTIVE: To assess the agreement in detecting and monitoring occlusal caries over thirty months using conventional visual and radiographic assessment and an intraoral scanner system which supports automated caries scoring. METHODS: Ninety-one young participants aged 12-19 years were included in the study. All occlusal surfaces were examined visually, radiographically (when indicated), and scanned with the TRIOS 4 intraoral scanner. TRIOS Patient Monitoring software (vers. 2.3, 3Shape TRIOS A/S, Denmark) was used for automated caries detection on the 3D digital models. RESULTS: Fifty-five of the study participants were re-examined after 30-months. Significant differences regarding caries detection were found between the conventional methods and the automated caries scoring system (p < 0.01), with moderate positive percent agreement (49-61%) and high negative percent agreement (87-98%). All methods reported significant caries progression over the follow-up period (p < 0.01). However, the automated system showed significantly more caries progression than the other methods (p < 0.01). CONCLUSIONS: The software for automated caries detection and classification showed moderate positive agreement and strong negative agreement with the conventional methods considering both the baseline and the follow-up assessments. The automated caries scoring system detected significantly fewer caries lesions and tended to underestimate the caries severity. All methods indicated significant caries progression over the follow-up period, while the automated system detected more caries progression. CLINICAL SIGNIFICANCE: The TRIOS system supporting automated occlusal caries detection and classification can assist in detecting and monitoring occlusal caries on permanent teeth as a complementary tool to the conventional methods. However, the operator should be aware that the automated system shows a tendency to underestimate the caries presence and lesion severity.

The aim of this study was to compare the assessment of tooth wear performed on digital models with the one conducted at the clinical examination. Seventy-eight volunteers (29 males and 49 females, age range 20-30 years) with at least 24 teeth, normal oral function, and a neutral transverse relationship were examined. During the clinical examination, dental wear was registered according to the Basic Erosive Wear Examination (BEWE) index. Subsequently, the BEWE index was blindly applied by two examiners on digital models obtained from the volunteers. Data were analyzed using weighted Cohen's kappa coefficient and correlation tests with a confidence interval of 95%. All volunteers showed signs of tooth wear. Anterior teeth showed increased severity of tooth wear than first molars. Early loss of tooth substance could be identified on the digital models, including in areas with challenging direct intraoral visual access. Approximately 50% of the scores based on clinical examination agreed with those based on examination of digital models (k = 0.543, p < 0.01). A moderate, positive correlation was observed between scores registered clinically and on digital models (Spearman's rho = 0.560, p < 0.001). Considering the rather low agreement between the clinical and digital scores, alternatives to using BEWE on digital models are needed.

OBJECTIVES: To examine the dimensional changes of endodontic sealers during 18 months using three-dimensional (3D) surface scanning and subtraction radiography in a novel in vitro sealer-extrusion model. MATERIAL AND METHODS: Fifty endodontically instrumented acrylic teeth were randomly allocated to five groups (n = 10) filled with Apexit Plus, AH Plus, BioRoot RCS, TubliSeal EWT, and gutta-percha (control). Freshly mixed sealers were intentionally extruded during obturation. All teeth were immersed in a physiologic solution for up to 18 months. Blinded 3D surface scans (resolution: ~10 µm) and digital radiographs of the teeth were obtained at baseline (immediately after obturation); and then after 1 week, and at 1, 3, and 18 months. For blinded assessment of sealer dimensional change, 3D models and radiographs were superimposed using specific software. Volumetric differences, root mean square (RMS), and area change from subtraction radiography measured at each period within each sealer group were thereafter calculated. Repeated measures analyses were done with Bonferroni adjustment for multiple comparisons; standard errors, p values, and 95% confidence intervals (CI) were reported. RESULTS: Analyses of the volumetric data confirmed significant, progressive material loss for Apexit Plus when compared to the other investigated sealers or the control group (p ≤ 0.02). Immersion period significantly influenced the volumetric dimensional changes of Apexit Plus already after 1 month (p < 0.01). For TubliSeal EW, the effect of the immersion period on the dimensional changes was noted after immersion for 3 months (p ≤ 0.02), while for BioRoot RCS this was evident only at 18 months (p < 0.01). Same trends were noted for the RMS data, whereas progressive dimensional changes using subtraction radiography only revealed significant changes for Apexit Plus (p = 0.01). CONCLUSIONS: The largest dimensional changes were shown by Apexit Plus, followed by Tubliseal EWT and BioRoot RCS. AH Plus remained stable throughout 18 months.

Purpose: To evaluate the marginal and internal fit of lithium disilicate and zirconia crowns using two optical coherence tomography (OCT) systems in order to estimate inter-system variations. Materials and methods: Ten lithium disilicate and 10 cubic stabilized zirconia crowns were placed on prepared artificial teeth without cement. Marginal discrepancy and internal cement gap of the crowns were assessed on images obtained using a swept source OCT (SS-OCT) and a spectral domain OCT (SD-OCT). Medians and interquartile ranges were calculated for both materials and OCT systems. Thereafter, Wilcoxon signed rank test was carried out. Results: No significant difference was found between the two OCT systems for absolute marginal discrepancy of either lithium disilicate (SS-OCT: 182 µm, SD-OCT: 214 µm; p = .922) or zirconia crowns (SS-OCT: 116 µm, SD-OCT: 121 µm; p = .232). Regarding internal cement gap, no significant difference was found between the two OCT systems for lithium disilicate crowns (SS-OCT: 128 µm, SD-OCT: 128 µm; p = .064). However, for zirconia crowns the SD-OCT showed significantly higher (p = .027) internal cement gap (92 µm) than the SS-OCT (68 µm). Moreover, it was not possible to assess the internal fit of zirconia crowns in 47% and 34% of the sites using SD-OCT and SS-OCT, respectively. Conclusions: No significant difference was noted in the ability of SS-OCT and SD-OCT to assess the marginal and internal fit of lithium disilicate crowns, nor the marginal fit of zirconia crowns. On the contrary, drawbacks regarding the assessment of internal fit of zirconia crowns using both OCT systems were observed.

OBJECTIVES: To assess the validity of an intraoral scanner system featuring near-infrared (NIR) transillumination to aid the detection of proximal caries lesions, and to compare the diagnostic performance of this system with that of conventional caries detection methods and with that of an intraoral camera featuring NIR transillumination (DIAGNOcam). METHODS: Ninety-five permanent posterior teeth were examined using a prototype tip functioning with TRIOS 4 intraoral scanner system (3Shape TRIOS A/S, Denmark) and emitting NIR light, DIAGNOcam, and visual and radiographic examination employing ICDAS criteria. One or two approximal surfaces per tooth, sound or with caries lesions at different stages, were examined (N1=158). Histological assessment was used as the reference standard. RESULTS: All methods showed excellent intra-examiner reliability (κintra ≥0.80). Two independent examiners assessed the NIR images obtained with both devices. The first examiner, who obtained and assessed the images, showed improved diagnostic performance than the second examiner, who only had access to the images. The inter-examiner agreement between the two examiners assessing the NIR images was substantial (κinter 0.57-0.72). The intraoral scanner and DIAGNOcam showed similar diagnostic performance. Regarding initial caries lesions, the NIR image assessment resulted in equal or improved sensitivity (SE 0.50-0.89) compared to radiographic assessment (SE 0.49-0.51) and higher than visual examination (SE 0.28-0.39). Radiographic and NIR image assessment resulted in similar SE in detecting moderate-extensive dentin caries lesions (SE 0.59-0.70), while visual examination showed an inferior value (SE 0.30). CONCLUSIONS: The intraoral scanner system featuring NIR transillumination and DIAGNOcam showed an overall good diagnostic performance. The conventional caries detection methods showed inferior sensitivity at initial caries lesion stages. CLINICAL SIGNIFICANCE: Considering the promising diagnostic performance of the intraoral scanner featuring transillumination and the advantages offered by combining the NIR images with the 3D models of the teeth, this system has the potential to contribute towards more reliable caries detection and monitoring in clinical practice without the use of ionizing radiation.

The use of 3D intraoral scanners (IOS) and software that can support automated detection and objective monitoring of oral diseases such as caries, tooth wear or periodontal diseases, is increasingly receiving attention from researchers and industry. This study clinically validates an automated caries scoring system for occlusal caries detection and classification, previously defined for an IOS system featuring fluorescence (TRIOS 4, 3Shape TRIOS A/S, Denmark). Four algorithms (ALG1, ALG2, ALG3, ALG4) are assessed for the IOS; the first three are based only on fluorescence information, while ALG4 also takes into account the tooth color information. The diagnostic performance of these automated algorithms is compared with the diagnostic performance of the clinical visual examination, while histological assessment is used as reference. Additionally, possible differences between in vitro and in vivo diagnostic performance of the IOS system are investigated. The algorithms show comparable in vivo diagnostic performance to the visual examination with no significant difference in the area under the ROC curves ([Formula: see text]). Only minor differences between their in vitro and in vivo diagnostic performance are noted but no significant differences in the area under the ROC curves, ([Formula: see text]). This novel IOS system exhibits encouraging performance for clinical application on occlusal caries detection and classification. Different approaches can be investigated for possible optimization of the system.

OBJECTIVES: To assess the feasibility of detecting and monitoring early erosive tooth wear using a 3D intraoral scanner (IOS) aided by specific software. METHODS: Extracted sound permanent teeth were assembled in two shortened artificial dental arches and scanned at different intervals with an IOS (3Shape TRIOS® 3) before and after an erosion/abrasion protocol (i.e. 1 h up to 24 h immersion in citric acid solution and subsequent brushing). The 3D models obtained at consecutive time points were superimposed with the baseline model using dedicated software (3Shape TRIOS® Patient Monitoring, version 2.1.1.0) and reference surface alignment. Surface profile differences between the baseline 3D model and the respective models from different time points were expressed as tooth substance loss. Non-parametric tests were used to assess the significance of tooth substance loss at different time points. Spearman's correlation was applied between the tooth substance loss at the end of each erosion/abrasion cycle and the immersion time in acid. RESULTS: Significant tooth substance loss (0.08 mm, IQR = 0.05) was detected by the software after 3 h of erosive-abrasive challenge (p = 0.045). The overall median loss increased gradually from baseline to 24 h showing a strong correlation with the immersion time in acid (rs = 0.971, p < 0.01). CONCLUSIONS: The use of an IOS aided by specific software showed good performance for early detection and monitoring of tooth wear in vitro and has promising potential for in vivo application. CLINICAL SIGNIFICANCE: Detection and monitoring of early erosive tooth wear can be reliably aided by intraoral scanning supported by specific software. The measurement error and uncertainty involved in this method should be taken into consideration when interpreting the tooth substance loss measurements. Furthermore, presuming the difficulty in defining reference surfaces in vivo, clinical validation is needed to determine the system's in vivo performance.

BACKGROUND: The potential of non-invasive optical coherence tomography (OCT) as a tool for assessment of fit of indirect reconstructions is not fully explored. OBJECTIVES: The objectives were to investigate the feasibility and validity of OCT, and to measure the internal and marginal fit of acrylic bridges fabricated using direct and indirect digitalisation. METHODS: The accuracy of the employed swept source OCT (wavelength: 1310 nm) was assessed by comparing with an object with known dimensions. Validity was assessed by measuring an OCT measurements on replica, mimicking the cement film thickness, with stereomicroscopic measurements. The reconstructions were placed on the abutments without cementation. The internal and marginal fit of acrylic bridges from direct and indirect digitalisation techniques were then assessed by obtaining 5 OCT B-scans per abutment tooth at pre-defined positions located 250 µm apart. The marginal and internal cement gaps were measured using image-processing software (ImageJ). Mean and standard deviation were calculated for both groups and t test assuming unequal variances was carried out. The level of significance was defined at 0.05. RESULTS: A strong linear correlation (r = 0.865) between OCT and stereomicroscopy was found. T test showed significantly (P < 0.01) better internal fit of bridges made from indirect digitalisation, but no difference in marginal fit. CONCLUSION: OCT is a feasible and valid tool for investigating internal and marginal fit of acrylic dental reconstructions. Better internal fit was observed in bridges fabricated using the direct digitalisation technique. No difference in marginal fit was found between the two fabrication methods.

Hydrogen dynamics in a time range from hundreds of femtoseconds to nanoseconds can be directly analyzed using neutron spectroscopy, where information on the inelastic and quasi-elastic scattering, hereafter INS and QENS, can be obtained. In this study, we applied these techniques to understand how the nanoscale mobility of the aqueous solution of polyacrylic acid (PAA) used in conventional glass ionomer cements (GICs) changes under confinement. Combining the spectroscopic analysis with calorimetric results, we were able to separate distinct motions within both the liquid and the GICs. The QENS analysis revealed that the self-diffusion translational motion identified in the liquid is also visible in the GIC. However, as a result of the formation of the cement matrix and its setting, both translational diffusion and residence time differed from the PAA solution. When comparing the local diffusion obtained for the selected GIC, the only noticeable difference was observed for the slow dynamics associated with the polymer chain. Additionally, over short-term aging, progressive water binding to the polymer chain occurred in one of the investigated GICs. Finally, a considerable change in the density of the GIC without progressive water binding indicates an increased polymer cross-linking. Taken together, our results suggest that accurate and deep understanding of polymer-water binding, polymer cross-linking, as well as material density changes occurring during the maturation process of GIC are necessary for the development of advanced dental restorative materials.

STATEMENT OF PROBLEM: Despite the increasing demand for a digital workflow in the fabrication of indirect restorations, information on the accuracy of the resulting definitive casts is limited. PURPOSE: The purpose of this in vitro study was to compare the accuracy of definitive casts produced with digital scans and conventional impressions. MATERIAL AND METHODS: Chamfer preparations were made on the maxillary right canine and second molar of a typodont. Subsequently, 9 conventional impressions were made to produce 9 gypsum casts, and 9 digital scans were made to produce stereolithography additive (SLA) casts from 2 manufacturers: 9 Dreve SLA casts and 9 Scanbiz SLA casts. All casts were then scanned 9 times with an extraoral scanner to produce the reference data set. Trueness was evaluated by superimposing the data sets obtained by scanning the casts with the reference data set. Precision was evaluated by analyzing the deviations among repeated scans. The root mean square (RMS) and percentage of points aligned within the nominal values (±50 µm) of the 3-dimensional analysis were calculated by the software. RESULTS: Gypsum had the best alignment (within 50 µm) with the reference data set (median 95.3%, IQR 16.7) and the least RMS (median 25.8 µm, IQR 14.6), followed by Dreve and Scanbiz. Differences in RMS were observed between gypsum and the SLA casts (P<.001). Within 50 µm, gypsum was superior to Scanbiz (P<.001). Gypsum casts exhibited the highest precision, showing the best alignment (within 50 µm) and the least RMS, followed by Scanbiz and Dreve. CONCLUSIONS: This study found that gypsum casts had higher accuracy than SLA casts. Within 50 µm, gypsum casts were better than Scanbiz SLA casts, while gypsum casts and Dreve SLA casts had similar trueness. Significant differences were found among the investigated SLA casts used in the digital workflow.

BACKGROUND: Cobalt release from dental prostheses has been shown to elicit allergic reactions in cobalt-allergic patients. It is therefore important to investigate whether these prostheses are possible sources of sensitization. OBJECTIVES: To assess (i) cobalt release from dental prostheses and (ii) allergic reactions to components of dental prostheses, and (iii) to investigate the oral mucosa for inflammation 1-5 years after insertion of the prostheses. METHOD: Clinical oral examination was conducted in 66 patients with 84 dental prostheses. Cobalt release from 84 functional (used) and 32 non-functional (new) prostheses was investigated with the cobalt spot test. Contact allergy was assessed by patch testing. Smear tests for Candida spp. were performed in patients showing signs of inflammation of the oral mucosa. The prostheses were assessed for biological and technical complications. RESULTS: None of the functional prostheses released cobalt, whereas this was observed in 24 of 32 non-functional prostheses. None of the patients had contact allergy to cobalt. Of the 66 patients, 11 showed signs of inflammation of the oral mucosa, 2 had oral candidiasis, 16 had ill-fitting prostheses, and all had insufficient oral hygiene. CONCLUSIONS: Dental prostheses released cobalt during the fabrication stages, but not 1-5 years after insertion. No allergic reactions were observed. Signs of inflammation were related to candidiasis, insufficient oral hygiene, and ill-fitting prostheses.

OBJECTIVE: To investigate the use of a three-dimensional (3D) digital scanning method in determining the accuracy of the wear performance parameters of resin-based composites (RBCs) determined using a two-dimensional (2D) analogue methodology following in-vitro testing in an Academisch Centrum for Tandheelkunde Amsterdam (ACTA) wear machine. METHODS: Specimens compatible with the compartments of the ACTA wear machine specimen wheel (n=10) were prepared from one commercial and four experimental RBCs. The RBC specimens were rotated against an antagonist wheel in a food-like slurry for 220,000 wear cycles. The mean wear depth was measured using the traditionally employed 2D and compared with the 3D profilometric (digital) techniques. Data were submitted to analyses of variance, Tukey's post hoc tests and Independent Samples Student's t-tests (where appropriate) at p<0.05. RESULTS: The RBC rank achieved for mean wear depth calculations were similar whether the 2D or 3D techniques were employed. However, the mean wear depth values obtained from the 3D digital technique were significantly increased for two of the five RBC materials compared with the 2D methodology. The total volumetric wear data provided an assessment of the potential of the experimental RBC formulations for clinical usage. CONCLUSION: The 3D technique allowed for the assessment of mean maximum wear depth and mean total volumetric wear which enables tribological analyses of the wear facet and therefore the wear mechanisms operative. Employing the 2D profile technique ranks RBC materials in terms of in-vitro wear performance. CLINICAL SIGNIFICANCE: Confidence in the wear volume measurements can only be achieved if the wear facet is analysed with sufficient resolution using a 3D digital measurement technique. However, the employment of 2D profilers is useful when screening potential new RBC formulations for the restoration of posterior dentition.

The development of dental materials with improved properties and increased longevity can save costs and minimize discomfort for patients. Due to their good biocompatibility, glass ionomer cements are an interesting restorative option. However, these cements have limited mechanical strength to survive in the challenging oral environment. Therefore, a better understanding of the structure and hydration process of these cements can bring the necessary understanding to further developments. Neutrons and X-rays have been used to investigate the highly complex pore structure, as well as to assess the hydrogen mobility within these cements. Our findings suggest that the lower mechanical strength in glass ionomer cements results not only from the presence of pores, but also from the increased hydrogen mobility within the material. The relationship between microstructure, hydrogen mobility and strength brings insights into the material's durability, also demonstrating the need and opening the possibility for further research in these dental cements.

OBJECTIVE: To investigate how the modulus of elasticity of resin composites influences marginal quality in restorations submitted to thermocyclic and mechanical loading. METHODS: Charisma, Filtek Supreme XTE and Grandio were selected as they were found to possess different moduli of elasticity but quite similar polymerization contraction. MOD cavities (n=30) were prepared in extracted premolars, restored and then subjected to thermocyclic and mechanical loading. Marginal quality of the restorations before and after loading was analyzed on epoxy replicas under a scanning electron microscope. The percentage of gap-free margins and occurrence of paramarginal fractures were registered. Modulus of elasticity and polymerization contraction were analyzed with parametric and margins with nonparametric ANOVA and post hoc Tukey HSD or Wilcoxon rank-sum tests, respectively. The number of paramarginal fractures was analyzed with exact Fisher tests (α=0.05). RESULTS: Grandio demonstrated significantly more gap-free enamel margins than Charisma and Filtek Supreme XTE, before and after loading (p<0.01), whereas there was no difference between Charisma and Filtek Supreme XTE (p>0.05). No significant effect of resin composite (p=0.81) on the quality of dentine margins was observed, before or after loading. Deterioration of all margins was evident after loading (p<0.0001). More paramarginal enamel fractures were observed after loading in teeth restored with Grandio when compared to Charisma (p=0.008). CONCLUSIONS: The resin composite with the highest modulus of elasticity resulted in the highest number of gap-free enamel margins but with an increased incidence of paramarginal enamel fractures. CLINICAL SIGNIFICANCE: The results from this study suggest that the marginal quality of restorations can be improved by the selection of a resin composite with modulus of elasticity close to that of dentine, although an increase in paramarginal enamel fractures can result as a consequence.

The purpose of this study was to provide the physicochemical interactions at the interfaces between two commercial etch-&-rinse adhesives and human dentin in a simulated moist bond technique. Six dentin specimens were divided into two groups (n=3) according to the use of two different adhesive systems: (a) 2-hydroxyethylmethacrylate (HEMA) and 4-methacryloxyethyl trimellitate anhydrate (4-META), and (b) HEMA. The Fourier transform infrared photoacoustic spectroscopy was performed before and after dentin treatment with 37% phosphoric acid, with adhesive systems and also for the adhesive systems alone. Acid-conditioning resulted in a decalcification pattern. Adhesive treated spectra subtraction suggested the occurrence of chemical bonding to dentin expressed through modifications of the OH stretching peak (3340 cm(-1)) and symmetric CH stretching (2900 cm(-1)) for both adhesives spectra; a decrease of orthophosphate absorption band (1040 to 970 cm(-1)) for adhesive A and a better resolved complex band formation (1270 to 970 cm(-1)) for adhesive B were observed. These results suggested the occurrence of chemical bonding between sound human dentin and etch-&-rinse adhesives through a clinical typical condition.

RESUMO

This paper presents the advantages and limitations related to the use of a clinical microscope in restorative dentistry, and it demonstrates the aid of magnification during preparation and restoration of a direct composite veneer. Good illumination and visibility is important to adequately viewing the adjacent dental tissues so that the resin composite buildup can mimic natural teeth. The reproduction of details results in a naturally esthetic direct veneer.

OBJECTIVES: The purpose of this study was to investigate the effect of light-curing protocol on degree of conversion (DC), volume contraction (C), elastic modulus (E), and glass transition temperature (T(g)) as measured on a model polymer. It was a further aim to correlate the measured values with each other. METHODS: Different light-curing protocols were used in order to investigate the influence of energy density (ED), power density (PD), and mode of cure on the properties. The modes of cure were continuous, pulse-delay, and stepped irradiation. DC was measured by Raman micro-spectroscopy. C was determined by pycnometry and a density column. E was measured by a dynamic mechanical analyzer (DMA), and T(g) was measured by differential scanning calorimetry (DSC). Data were submitted to two- and three-way ANOVA, and linear regression analyses. RESULTS: ED, PD, and mode of cure influenced DC, C, E, and T(g) of the polymer. A significant positive correlation was found between ED and DC (r=0.58), ED and E (r=0.51), and ED and T(g) (r=0.44). Taken together, ED and PD were significantly related to DC and E. The regression coefficient was positive for ED and negative for PD. Significant positive correlations were detected between DC and C (r=0.54), DC and E (r=0.61), and DC and T(g) (r=0.53). Comparisons between continuous and pulse-delay modes of cure showed significant influence of mode of cure: pulse-delay curing resulted in decreased DC, decreased C, and decreased T(g). Influence of mode of cure, when comparing continuous and step modes of cure, was more ambiguous. SIGNIFICANCE: A complex relationship exists between curing protocol, microstructure of the resin and the investigated properties. The overall performance of a composite is thus indirectly affected by the curing protocol adopted, and the desired reduction of C may be in fact a consequence of the decrease in DC.

OBJECTIVES: The purpose of this study was to investigate the effect of light-curing protocol on softening and elution of monomers in ethanol as measured on a model polymer. It was a further aim to correlate the measured values with previously reported data on degree of conversion and glass transition temperature for the same polymer and curing protocols. METHODS: Different light-curing protocols were used in order to investigate the influence of energy density, power density, and mode of cure on the properties of a model polymer. The modes of cure were continuous, pulse-delay, and stepped irradiation of the specimens. Wallace hardness was used to determine the softening of the polymer after storage in ethanol for 24h. Elution of monomers from the polymer was assessed after 7 days in ethanol by means of high-pressure liquid chromatography (HPLC). Data were submitted to two- and three-way analysis of variance (ANOVA), Newman-Keuls' multiple comparison test, and linear regression analysis. RESULTS: Energy density, power density, and mode of cure of the polymer influenced the softening and elution of monomers in ethanol. As energy density increased, softening and elution in ethanol decreased. At same energy density, the influence of power density varied with the mode of cure. When compared to the continuous mode of cure, and at same energy density, pulse-delay irradiation resulted in polymers that in general were more susceptible to softening, but eluted monomers to a lower extent. Less elution was also found with step-cured polymers. Significant, negative correlations were detected between softening and elution in ethanol, respectively, and degree of conversion and between softening and elution in ethanol, respectively, and glass transition temperature. SIGNIFICANCE: A complex relationship exists between curing protocol and the properties selected for investigation. The effect of different combinations of exposure periods and power densities are important to understanding how the curing protocol affects the properties of polymer-based materials.
